Welcome to your new residence! It is the responsibility of every resident to complete and submit this condition form within one week (7 days) of check-in, a room change, or after Rotation. Please Note: If there are any changes in living accommodations, a new Room Condition Form must be submitted to the Housing Office (within 7 days of the change).
This form is for Undergraduate on-campus residents only! Do not use this form if you live off-campus or in the Graduate Catalina apartments.
The room condition form is for your protection. Your room will be checked for damages at the time that you check-out of your room or if you change rooms. To eliminate any charge for missing items or damages that you have not caused, please inspect the items and be specific in indicating the extent of damage and the exact location. Among the things you should look for are dust, grease, holes, stains, burns, damages and wear. Not all items listed are standard for every room.
Off Campus Room/Unit Condition Form
This form is for residents of furnished lottery Graduate Catalina apartments and furnished or unfurnished Graduate off campus lottery housing locations. This form will be kept on file to note the condition of the unit when you take occupancy. You may be held responsible for any changes or damages that occur during your occupancy other than normal wear and tear, so make sure to complete the form thoroughly. When submitting the form, check the premises carefully and be specific. Use the comments field to add additional items if necessary.
To protect yourself from unwarranted charges, the premises must be left in the same condition as when you moved in. A pre-move out inspection will be scheduled prior to your departure in order to determine what, if any, corrections you will need to make to avoid damage charges.
This inspection should help insure against any additional charges. The preliminary move-out inspection will be done within and no sooner than 14 days prior to your departure. The final move-out inspection will be performed after your departure by a Maintenance Inspector.
